Report on the experience of clinical use of the «ALMAG-02» magnetic therapy device

Introduction

In the period from 2014 to 2020, on the basis of the physiotherapy department SBHI «City Hospital №37 of the Avtozavodsky district of Nizhny Novgorod» a magnetic therapy device «ALMAG-02» according to GICS.941519/104 TU was applied, designed for physiotherapy with a low-frequency, low-intensity magnetic field in the treatment of a wide range of diseases. The department is equipped with three devices "ALMAG-02" (10294 -2014,10302-2014,16229- 2016), with the help of which courses of physiotherapy are carried out to 50-70 patients daily. Magnetic therapy from the Almag-02 device was used for various groups of patients undergoing rehabilitation after fractures, dislocations, ligamentous injuries, soft tissue bruises, as well as physiotherapy courses for diseases of the respiratory system, ENT organs, diseases of the peripheral nervous system, etc.

Methods of application of «Almag-02»

During the procedures, the contact method of exposure was used, the inductors of the device were installed on the area of the affected organ. The main emitter (ME) was used, consisting of 4 flexible radiating lines with 4 inductors each, an emitter in the form of a separate flexible ruler (SFR) and local emitters (LE) containing 2 inductors. Structural embodiment of the device is convenient and allows you to use it for local and zonal treatment, that is, to simultaneously affect several zones with a magnetic field. LE provide only a local pulsed magnetic field with a large penetration depth. The device is programmed for 79 programs with optimally selected characteristics of the magnetic field. Field types: "running" and " stationary". On the emitters of the device there is a marking of the polarity of the field (N and S). Exposure time from 10 to 30 minutes. with good portability. The procedures were performed daily from 10 to 20 per course. The nosological structure of the groups of patients who received magnetic therapy from the «ALMAG-02» device is presented below.


Neuropathy is an inflammatory or degenerative change in the trunks of the peripheral nerves, accompanied by sensitive, motor and trophic disorders in the innervation zone of the damaged nerve. When the peroneal nerve was affected, the patients were characterized by a violation of finger flexion, the development of atrophy of the muscles of the posterior surface of the lower leg. In the neuropathy of the sciatic nerve, atrophy of the muscles of the posterior surface of the thigh was attached. Patients complained of pain in the lower back, in the leg, along the nerves, they had difficulty in movement, gait disorders, and the development of muscle paresis. Magnetic therapy with the almag device had an anti-edematous, anti-inflammatory, analgesic effect by improving microcirculation in the affected area , relieving muscle spasm. The effect of treatment was manifested quite quickly. The assessment was conducted on the VAS scale.

In this group of diseases, the method of magnetic therapy was widely used. For example, epicondylitis is a chronic dystrophic lesion of the external or internal condyles of the humerus, accompanied by aseptic inflammation of the muscles at the place of their attachment to the bone in the area of the elbow joint. Epicondylitis occurs due to microtraumas, deforming arthrosis of the elbow joint, cervical osteochondrosis, and ulnar nerve neuropathy. Clinically, epicondylitis is characterized by pain of varying intensity in the area of the affected epicondyle radiating into the forearm. «Almag-02» magnetic therapy was applied to provide anti-inflammatory and analgesic effects. separate flexible ruler (отдельная гибкая линейка) Program № 31 was chosen: a separate flexible ruler emitter was used for the affected elbow, running MF, induction of 20 mT, frequency of 100 Hz, exposure time of 15 minutes.

Plantar fasciitis is characterized by the presence of bone-cartilaginous exostases in the area of the calcaneal tubercle. With the worsening of disease, patients experienced sharp pain in the heel area. The effect of the magnetic field (program №36) provided a reduction of pain syndrome, swelling, and X-ray and ultrasound examination revealed a decrease in signs of an inflammatory reaction and a discharge of calceficates. A good therapeutic effect was revealed when using a magnetic field (program 32) in acute trophoneurotic bone atrophy (Zudek's syndrome). This is a severe complication of fractures of the radius in a typical place, accompanied by severe edema, pain syndrome, numbness of the fingers. The magnetic field affected the rheological properties of the blood, had pronounced analgesic, anti-inflammatory, decongestant and trophostimulating effects, normalized microcirculation and venous blood flow.

Treatment of open extensive injuries of soft tissues and bones is one of the most difficult problems of surgery and traumatology. Laboratory and experimental studies have proved that the low-frequency magnetic field has a bacteriostatic effect on the pathogens of wound infection and stimulates reparative processes in damaged tissues, so the method has found wide clinical application both in the hospital and in outpatient settings. When comparing the results of treatment in the main and control groups, a positive effect of the magnetic field on the course of reparative processes in the wound was noted. Purulent wounds were cleared of necrotic tissues 2-3 days faster, the phenomena of perifocal inflammation disappeared, epithelization occurred earlier than in patients who did not receive magnetic therapy. The use of the magnetic field from the ALMAG device has proven itself well in the treatment of bruises of various localizations,injuries of the capsular-ligamentous apparatus of the joints and other injuries. In patients receiving magnetic therapy, there was a more rapid reduction in pain syndrome, edema, and, as a result, improved mobility in damaged joints.

In the control group that did not receive magnetic therapy, pain syndrome regression was observed in 17% of patients by the 5th day of treatment, in 41% by the 10th, in 65% by the 15th, and in 80% by the 20th day. In the group where magnetic therapy from ALMAG was used, a positive effect of its effect was observed in the form of a reduction in pain syndrome in a larger number of patients: on the 5th day, the regression of the pain syndrome was observed in 40% of patients, on the 10th-in 65%, on the 15th-in 84%, on the 20th-in all patients.

In diseases of the respiratory system, in particular viral and bacterial pneumonia, the magnetic field was applied from 10-12 days and provided an immunomodulatory, resorbing effect. The main emitter was placed on the surface of the chest. Program 20 was chosen. The use of this technique provided an acceleration of the recovery process and reduced infiltration by 3-5 days compared to patients who did not receive magnetic therapy (according to X-ray data).

Lung tissue damage in patients receiving magnetic therapy from ALMAG decreased by 43% by day 12, and infiltration disappeared by day 24.

In the control group that did not receive magnetic therapy, by the 12th day there was a decrease in infiltration by 25%, by the 24th day by 85%.

In exudative pleurisy, physical methods were included in the complex treatment after the exclusion of the tumor genesis of pleurisy and the removal of exudate from the pleural cavity. Programs №22, №23 were used. As a result, there was an improvement in the drainage function of the lungs, sputum discharge (due to the antispasmodic effect), and new fluid accumulations were prevented.

The magnetic field was included in the complex treatment with anti-inflammatory, anti-edematous purpose, had a positive effect on the restoration of the vocal function of the larynx. In eustachitis and sensorineural hearing loss, there was a positive momentum in the form of increased hearing acuity, confirmed by audiogram data.

Dyscirculatory encephalopathies can have an atherosclerotic, hypertensive, post-stroke, toxic, post-traumatic nature, and can also be of a mixed type. Along with medical therapy, physical methods were used, and the factor of choice was magnetic therapy on the cervical-collar zone to improve blood supply in the vertebrobasilar pool, for normalize the processes of excitation and inhibition, and to influence brain tissue. We used the main radiator for the cervical-thoracic region, the radiator in the form of a separate flexible ruler was wrapped around the head (program №16).

The therapeutic effect was obtained with all patients in the form of reducing dizziness, headaches, stabilizing blood pressure, improving the emotional background, working capacity.

Slight improvement was observed in patients due to the severity of the disease, co-morbidities, and age.

The magnetic field from ALMAG has found wide application in obliterating diseases of the vessels of the lower extremities (program №17). The patients had ischemic syndromes (insufficiency of blood supply), they complained of pain of varying intensity, weakness in the legs, a sensitivity to cold, numbness during walking and at rest. Physiotherapy was most effective in patients with stage 1 and 2 lower leg and foot ischemia. With regular courses of magnetic therapy (2-3 times a year), patients noted a decrease in these symptoms, an increase in adaptation to physical exertion and a decrease in pain syndrome according to the VAS system.

Raynaud's disease does not belong to occlusive diseases, it is angeotrophoneurosis with damage to small arteries and arterioles of the extremities as a result of angiospasm. Physical therapy techniques were also effective in treating patients with this disease ( program №11).

The magnetic field had a normalizing effect on the anticoagulation system of the blood, improved blood flow, reduced the possibility of parietal thrombosis, improved microcirculation in the capillaries, stimulated the formation of new colloterals, had a decongestant, anti-inflammatory, analgesic effect compared to the control group, where magnetic therapy was not used.

Physical methods were most often used in the complex treatment of patients with acute chronic diseases of the uterine appendages. There was a direct dependence of the effectiveness of treatment of salpingitis and oophoritis on the timely connection of magnetic therapy. The program 78 was used. Analyzing the results of treatment of patients, it was found that electrotherapy was prescribed in 50.2 %, magnetic therapy in 78.8%, laser therapy in 48.5% of patients. Courses of magnetic therapy for diseases of the pelvic organs were evaluated as highly effective due to the decongestant, resorption, antispasmodic action. Against the background of treatment, pain and discomfort in the lower abdomen decreased, the menstrual cycle normalized, and the blood picture normalized. According to the long-term results of ultrasound, a decrease in adhesive processes was recorded.

As a result of the use of magnetic therapy, the symptoms of the disease regressed after an average of 20 days. The joint function was restored. In the control group of patients who received only medical treatment, the restoration of joint functions and the regression of inflammation occurred 10-15 days later.

In the presence of a keloid scar with the use of magnetic therapy, there was a decrease in perifocal inflammation, softening of the scar tissue. When using other methods of physiotherapy (laser, ultrasound), the effect of magnetic therapy was enhanced.

Conclusion

The method of low-frequency pulsed magnetic therapy from the «ALMAG-02» device is highly effective in the treatment of various groups of diseases. In all cases of clinical use of the device, more pronounced therapeutic effects were obtained, demonstrating improvements in the function of the affected organ, than in cases where magnetic therapy was not included in the treatment or rehabilitation complex.

Magnetic therapy does not cause the formation of endogenous heat, is well tolerated, without causing adverse side effects, even in comorbid patients and elderly persons. The device is safe and easy to use. Due to the presence of several types of emitters in the «ALMAG-02» device, it becomes possible for zonal use, which affects the acceleration of obtaining a therapeutic effect.

Thus, the use of magnetic therapy in the complex therapy and rehabilitation of various diseases helps to improve the results of treatment and reduce the recovery time of patients.

The obtained results of the practical application of the «ALMAG-02» device confirm its effectiveness and safety in the complex treatment of a wide range of diseases.
